This policy covers information collected through avedentistry.com. Your dental records and anything you tell us in the office are covered separately by our HIPAA Notice of Privacy Practices, which we provide to every patient. Ask at the front desk for a copy.

Information you give us
When you fill in the appointment request or contact form, we collect the name, phone number, email address and any message you enter. We ask you not to include medical history, insurance numbers or other sensitive details in web forms, because email is not a secure channel. We collect that information by phone or in person instead.
Information collected automatically
Like most websites, this site records basic technical information when you visit: your approximate location based on IP address, the browser and device you used, which pages you viewed, and how you arrived at the site. This is used to understand how the site is performing, not to identify you personally.

Each of these operates under its own privacy policy. Following a link away from this site, for example to CareCredit or to Google to leave a review, means that site's policy applies rather than ours.
How we protect your information
This site uses HTTPS encryption. Form submissions are sent to our practice email and are accessible only to staff who need them. That said, no method of transmitting information over the internet is completely secure, which is the reason we ask you to keep medical and insurance details out of web forms.
How long we keep it
Form submissions are kept as long as needed to respond to your request and to maintain a record of contact. Analytics data is retained according to Google's default retention settings. Dental records are governed by California law and by our HIPAA Notice of Privacy Practices, not by this policy.
Your privacy rights in California
Under California law you can ask what personal information we hold about you, ask us to correct it, and ask us to delete it. Call 408-779-2129 or write to us at the address above and we will respond.
We do not sell personal information, and we do not share it for cross-context behavioral advertising. We will not treat you differently for exercising any of these rights.
Note that medical and dental records carry their own retention requirements under California law and under HIPAA, so a request to delete website information does not extend to your clinical records.
Children
This website is not directed at children under 13 and we do not knowingly collect personal information from them through it. Parents and guardians book appointments for children, and any information they provide is treated as their own submission. If you believe a child has submitted information through this site, call 408-779-2129 and we will remove it.
Links to other sites
Where this site links to another, such as CareCredit or Google, that site's privacy policy governs what happens there. We do not control those sites and are not responsible for their practices.
Changes to this policy
If we change how this site handles information, we will update this page and change the date at the top. Significant changes will be noted clearly rather than made quietly.
